To help us provide you with the best possible care, please remember to bring your government-issued ID. Kindly complete all your new patient forms prior to your appointment. If applicable, please bring your dental insurance information. This will streamline the billing process and maximize your benefits. If you have any previous dental records or X-rays from another provider, feel free to bring them along. They can be valuable for reference and comparison.
Additionally, it's helpful to have a list of any medications you're currently taking. This information can aid in assessing your overall health and ensuring safe dental treatments. We want your visit to be as efficient and comfortable as possible, so having these items on hand will ensure a smooth check-in process. For your convenience parking is validated by us for 90 minutes. The parking entrance is located at 900 N La Brea Ave.
